Coming home safely, and getting stronger

Leaving hospital is a big step, and the first days at home matter most. Our carers are ready from the day of discharge to help you settle, stay safe and start rebuilding your independence.

We work closely with ward teams and families to plan everything, so you come home with the right support in place and a clear plan to get back to doing more for yourself.

What our discharge support includes

Two ways we can bring you home

Straight home with care visits: carers ready at your own home from the day you are discharged, with support that reduces as you recover.

A step-down stay in our accommodation: if home is not quite ready, you can recover in our homely supported living accommodation first, then move home with care in place.
